At ALL Heart Mobile Veterinary Services we strive to provide the best care to your pet and best customer service to you.  With that in mind, we offer limited telemedicine consults to our established clients.

REQUIREMENTS for a Telemedicine Consult include the following:

  1. A completed telemedicine waiver dated for the day of the appointment.
  2. The consult must be with a doctor employed at AHMVS that your pet has seen in the last 6 months.
  3. Pictures, videos, and any previous records that support the reason for the consult must be sent to AHMVS to be placed in your pet’s medical record before the scheduled consult for doctor review.It is vital that you give as much information as you have. You never know what could be important.
  4. An appointment must be scheduled with the doctor on the case to reach out and discuss your pet’s case with you.

The doctor on the case will review the history you provide along with any other documentation you provide and decide if he/she can move forward with a telemedicine consult.  Unfortunately, diagnosis and treatment with telemedicine is very limited in veterinary medicine.  Please understand that a telemedicine consult does not equal the benefits of a good physical exam and therefore issues relevant to a proper diagnosis could be missed.

The doctor on the case has the exclusive right to determine if a telemedicine consult is appropriate for making a diagnosis or if your pet should be physically seen for an exam.

Telemedicine consults cannot be scheduled in place of regular lab work monitoring for prescription refills.

Telemedicine Consult Waiver

  • Telemedicine involves the use of electronic communications to enable a veterinarianemployed by ALL Heart Mobile Veterinary Services at a different physical location to provide medical advice for an individual patient whom he/she has a Veterinarian-Client-Patient relationship established.


    Possible Risks:

    There are potential risks associated with the use of telehealth. These risks include, but may not be limited to:
    • In some cases, the information provided may not be sufficient (e.g., poor resolution of images) to allow for appropriate decision making by the veterinarian.
    • Delays in physical evaluation, diagnostic testing, and treatment could occur due to the limited nature of telemedicine which could result in exacerbation of the known issue as well as secondary sequalae.
    • In some cases, a lack of access to complete health records may result in interactions or allergic reactions or other judgment errors.
    By signing this form, I understand the following:
    1. I understand that the laws that protect privacy and the confidentiality of health information also apply to telehealth, and that no information obtained in the use of telehealth which identifies me or my pet will be disclosed to researchers or other entities without my consent.
    2. I understand that I have the right to withhold or withdraw my consent to the use of telehealth in the course of my pet’s care at any time, without affecting my right to future care or treatment for my pet.
    3. I understand that the veterinarian assigned to my pet’s case has the right to withdraw consent to the use of telehealth in the course of my pet’s care if that veterinarian does not feel telemedicine is appropriate for my pet.
    4. I understand that I have the right to inspect all information obtained and recorded in the course of a telehealth interaction, and may receive copies of this information for a reasonable fee.
    5. I understand that a variety of alternative methods of health care may be available to me, and that I may choose one or more of these at any time. My veterinarian has explained the alternatives to my satisfaction.
    6. I understand that it is my duty to inform my veterinarian of any and all previously diagnosed issues with my pet.
    7. I understand that no results of the advice provided through telemedicine can be guaranteed or assured.
